Quality Problem Solving

Which Root Cause Analysis Method Should You Use?

The right root cause analysis method depends on the problem structure, the evidence available and the level of control the investigation requires. A method can provide a useful starting point without being the only method a team uses.

    Start with the problem structure

    SituationTypical fitWhy
One focused causal path5 WhyIt follows one evidence-backed chain in depth.
Several possible cause familiesFishboneIt makes broad cross-functional exploration visible.
Interacting events or explicit alternativesFTAIt models AND/OR event logic.
Complaint needing containment through closure8DIt governs the wider corrective-action investigation.

    Consider evidence and scope

If facts are sparse, first define the problem and collect evidence before treating any method output as a cause. If the issue affects a customer, needs containment, crosses functions or needs effectiveness verification and prevention, the analysis belongs within a broader 8D process.

A method can be combined with another method. For example, a team may use Fishbone to expose candidate cause families, 5 Why to investigate one branch, and FTA to test whether a top event requires several conditions. The chosen technique should fit the question, not replace disciplined verification.

    Avoid universal rules

“Use 5 Why for simple problems” is useful shorthand, not a law. A seemingly simple defect can involve multiple shifts, suppliers or system interfaces. Likewise, a sophisticated FTA is not automatically better if the available evidence cannot support its branches.

Choose the smallest method that makes the problem structure visible, then expand the investigation when facts require it. Keep candidate causes separate from proven causes until observation, records, testing or controlled verification establish the causal mechanism.

    When to use 8D

Use 8D when the organization needs a controlled investigation: team ownership, customer protection, containment, root-cause verification, corrective action, effectiveness checks, prevention and formal closure. 8D may use one or more RCA methods; it does not require every method.

Use the method within the right level of control

For a complaint that needs containment, evidence, team ownership, corrective action, effectiveness verification and prevention, run the wider investigation in Qhubio 8D.
