What is the 5 Why method?
The 5 Why method is an iterative questioning technique used in root-cause analysis. You start from a clearly stated problem and repeatedly ask why the previous answer occurred, building a causal chain from the visible symptom towards a controllable cause.
It originated as a practical shop-floor problem-solving method and is now widely used in quality management systems such as ISO 9001 and IATF 16949. The number five is a guideline, not a mathematical rule — some problems resolve in three steps, others need more.
How to use the 5 Why method
- State the problem as an observed fact, with the part, process and defect clearly identified.
- Confirm the problem with data: quantity, timing, location and detection point.
- Ask why the problem occurred and answer with a cause you can verify, not an opinion.
- Ask why that cause occurred and repeat until the answer describes a controllable condition.
- Test the chain backwards: each cause must plausibly produce the effect above it.
- Verify the root-cause candidate with evidence before defining corrective action.
- Confirm effectiveness after implementation and standardise the control.

The final answer is a root-cause candidate. It is confirmed only when evidence shows that introducing a verified seating control removes the failure, and that removing the control reproduces it.
When 5 Why is not enough
- Multiple interacting causes contribute to the same failure.
- Complex systems where the causal path branches instead of running in a single line.
- Insufficient evidence, so each answer is an assumption rather than a verified fact.
- Recurring customer complaints that already survived a previous corrective action.
- Cross-functional problems requiring containment, validation and prevention steps.

5 Why in an 8D investigation
In an 8D report, root-cause analysis happens in D4. 5 Why is one of the accepted techniques used there, usually together with evidence review and cause verification. The chain you build here can be carried into D4 as the causal hypothesis, then confirmed against the data collected in D2.

A Fishbone (Ishikawa) diagram maps many possible cause categories — man, machine, method, material, measurement, environment — in breadth. 5 Why works in depth along one causal line. In practice, teams use a Fishbone to identify candidate causes, then a 5 Why chain to follow the most likely candidate down to a controllable condition.
