                What is a Fishbone Diagram?

                A Fishbone Diagram — also called an Ishikawa diagram or cause-and-effect diagram — is a structured brainstorming tool used in quality engineering. The problem, or effect, is placed at the head of the diagram, and the potential causes are grouped into cause categories that branch off a central spine.

                The value of the method is coverage: instead of jumping to the first plausible explanation, the team works systematically through several families of potential causes before deciding what to investigate.

                How to create a Fishbone Diagram

                1. Define the problem as an observed fact, without embedding an assumed cause.
                2. Choose the cause categories that fit the problem — the 6M set is a common default.
                3. Brainstorm potential causes in each category with the people who know the process.
                4. Add deeper contributing factors below a cause where the mechanism needs more detail.
                5. Review gaps and interactions between categories.
                6. Verify the most important causes with evidence before treating them as root causes.

                When a Fishbone Diagram is not enough

                A Fishbone Diagram organises hypotheses; it does not verify them. Once the candidate causes are on the diagram, the investigation continues with 5 Why analysis, designed experiments, data analysis, process observation or a full structured 8D investigation.
