    Severity, Occurrence and Detection

    Severity describes the consequence if the failure reaches the customer or process. Occurrence estimates how often the cause is expected. Detection reflects the ability of current controls to detect the issue. Use Qhubio’s FMEA rating guides to anchor each rating consistently.

    How FMEA Action Priority differs from RPN

    RPN multiplies Severity × Occurrence × Detection. Action Priority is a decision logic: it deliberately gives seriousness and detection context more weight than a single multiplication result. Similar RPN values can therefore lead to different Action Priorities.

    Worked-use example and limitations

    For a torque-control concern, enter ratings supported by process evidence, calculate the result and use it to decide whether action is needed. Do not treat a calculator result as a public decision table, a replacement for engineering judgement or a complete FMEA record.
