The first row is the canonical reason AIAG-VDA replaced RPN: a safety failure that an arbitrary 150 threshold filters out. Action Priority forces it to High because Severity is 10.
RPN = Severity × Occurrence × Detection. Teams used a threshold (often > 100) to decide which failure modes required action. The simplicity of one number made it easy to sort and report — and easy to game.

AP weighs Severity first. High Severity always yields elevated priority regardless of how low Occurrence and Detection appear. AP is a rule table, not a calculation, so it cannot be gamed by inflating Detection confidence. See the full logic in the Action Priority Guide.
