Risk Matrix vs FMEA: Which Should You Use?

Use a risk matrix to prioritise broad exposure using likelihood and impact. Use FMEA when you need to examine how a product or process can fail, connect effects and causes, and evaluate prevention and detection controls. Choose the method by the decision you need to make.

A matrix helps a team ask, ‘Which exposures need attention?’ Process FMEA asks, ‘How could this process function fail, what would happen, why could it happen, and how do our controls address it?’ A register is the working record around the broader risks; it is not itself a replacement for detailed failure analysis.

When the risk matrix is a useful starting point
Start with a matrix when the team needs a shared overview across different exposures. A supplier transition might include late delivery, equipment availability, training gaps and dimensional quality. A register helps assign owners and keep follow-up visible without forcing every topic into a failure-mode worksheet.
The matrix is especially useful for early discussion when detailed process knowledge is incomplete. Document uncertainty and identify what evidence is needed. A quick assessment should help focus the next investigation, not hide gaps behind a precise-looking number.
When you need Process FMEA
Use Process FMEA when you need to distinguish specific failure scenarios and their technical controls. A register entry for incorrect assembly torque can be expanded into causes such as an incorrect setting, missed tightening, tool drift or joint variation. Their prevention controls and ability to detect the failure may differ.
FMEA is also appropriate when the customer or applicable process requires a structured FMEA deliverable. Confirm the required methodology and customer-specific expectations. A generic matrix PDF does not meet a requirement for a complete AIAG-VDA Process FMEA merely because both documents discuss risk.
Worked handoff: from dimensional risk to PFMEA
The broad assessment identifies bore drift as a priority exposure in a CNC cell. The team then studies the finishing step in PFMEA: the intended function is to achieve the specified bore; the failure mode is an out-of-tolerance diameter; the effect might be loss of assembly fit.
Investigate specific causes and controls instead of copying the matrix score. Use evidence to justify Severity, Occurrence and Detection, then calculate AP. The register can reference the PFMEA work and its control improvements. The handoff is an engineering task; Qhubio does not promise automatic transfer from the free register into a generated FMEA.
Where the free Action Priority calculator fits
The AP calculator supports one S/O/D combination at a time and shows AP alongside the legacy RPN. It is useful while learning the rating logic or checking a single scenario, but it does not identify your failure modes or validate the ratings you entered.
A matrix score is not RPN, and a matrix colour is not Action Priority. The AIAG-VDA approach introduced AP in place of RPN-based prioritisation. For many process steps and failure scenarios, use the complete FMEA record so causes, controls, actions and rating rationales stay together.
